The purchaser of Natural and Theatrical special effect contact lenses acknowledges and agrees to the following:

You must get an eye exam prior to ordering your Natural or Theatrical special effect contact lenses, if you have a prescription it must be at least 12 months current. It is highly recommended that after purchasing your Natural or Theatrical special effect contact lenses you return to your eye doctor to be instructed on how to put them in and take care of them properly. You’re Natural or Theatrical contact lenses can be worn for up to 10 hours a day. All orders must be paid in full to be processed. All sales are final, no refunds.

Please! Do not share your Natural or Theatrical special effect contact lenses with anyone! Contact lenses, even if used only for theatrical purposes are registered as a medical device. Sharing your Natural or Theatrical special effect contact lenses can spread micro-organisms between people that can result in serious eye health problems.

- That he or she is at least 18 years of age. If under 18, this form must be filled out and signed by a parent or legal guardian.

- Natural and Theatrical contact lenses are only warranted or guaranteed for fading. FX Eyes is not liable for breakage or malfunction. If fading occurs we touch them up at no charge.

- Agrees and acknowledges this device is for Natural and Theatrical use only.

- FX Eyes or any of it's employees, agents, or representatives shall not be held liable by the purchaser against any claim, loss, damage or personal injury caused by wearing Natural and Theatrical special effect contact lenses whether to the purchaser or any other third party.
